Home health care typically includes clinical services such as wound care, medication administration, or physical therapy provided by a registered nurse or licensed therapist.
At MHV WellCare, our caregivers provide steady and supporticompanionship and assist with everyday routines such as meal preparation, light housekeeping, medication reminders, gentle movement. We also provide verbal cues and encouragement around personal routines, helping them to feel confident and stay independent at home.
Independent caregivers may offer lower rates but often work without the structure, supervision, or safeguards of agency-based care. At MHV WellCare, all caregivers are carefully vetted and NYS certified—either as Personal Care Aides, Home Health Aides, or in a related health or caregiving field. Each caregiver is supported by a team that includes dedicated case coordinators and a registered nurse who oversees every care plan. We are fully insured and bonded for everyone's safety and protection.
All of our services are provided on a private pay basis, directly between MHV WellCare and our clients or their families. Clients with Long-Term Care Insurance might be eligible for reimbursement, and we can support you in that process by providing detailed invoices and a Plan of Care (POC) that is prepared and overseen by our nursing staff. This documentation often helps meet the requirements that insurance companies look for when reviewing claims.

Click here to set up a free in-home consultation. During this visit, we’ll discuss your needs and explain how our services can help. If we determine that we’re a good fit, one of our nurses will conduct a formal intake at your home. Together, you’ll create a personalized care plan that outlines the specific services we’ll provide, along with the days of the week and number of hours per day you’ll receive care. We review all care plans quarterly to ensure they continue to meet your needs.
Once you have been onboarded as a client, we will send an email to you and anyone you authorize with login credentials to access the Family Portal. To access the portal and see your care plan and communicate with your care team, simply enter your username and password in the indicated boxes and press the login button. The website has a helpful tutorial that shows how to take advantage of all Family Portal features.
We currently offer three ways for clients to pay for services:

Or email mycaremanager@mhvwellcare.com with the client’s name and the date/time of the visit you’d like to change or cancel.
When we hire caregivers, we create an in-depth profile that includes their skills, specialty training, interests, career goals, work history, personality type, and availability. Once your intake is complete, we can quickly identify team members who are both well-matched to your needs and available to help. We’ll then introduce you to your caregiver and care coordinator during a first visit in your home, so everyone has a chance to get to know each other.
If for any reason you’re not completely satisfied with the care you’re receiving from MHV WellCare, please contact your care coordinator or reach out directly to our owners, Todd and Erik, at 1-838-265-2935 (option 2). We’ll get back to you within 24 hours and work with you to address the issue — whether that means adjusting your care plan, providing additional training or support for your caregiver, or matching you with someone else on our team.
